Female to male top surgery creates a masculine chest contour by removing the breast (referred to as subcutaneous mastectomy). A variety of surgical approaches are available to achieve this goal, and may utilize free nipple-areola grafts, nipple-areola resizing and reposition, and/or liposuction to achieve the best result. Top surgery is the most commonly performed gender reassignment surgery, allowing patients to live more comfortably in their preferred gender role, thereby improving psychological and social functioning. Oftentimes, top surgery is the only surgical procedure performed as part of the transition.
Depending upon your anatomy, Dr. Blechman will explain which surgical approach is bested suited for you. Patients with smaller breasts may be candidates for an incision only around the areola, at which time the areola can often be resized. Patients with larger breasts may require longer incisions across the breast (sometimes referred to as a double incision technique), leaving a scar along lower border of the pectoralis muscle. The double incision technique generally involves using a free nipple-areola graft. Regardless of what surgical approach is used, liposuction is frequently employed to help contour a more masculine chest. The procedure is performed under general anesthesia and takes approximately three to four hours. Patients return home that same day.
Bruising and swelling are normal and may take weeks to resolve. Stitches are generally placed beneath the skin and do not need to be removed. If free nipple-areola grafts were placed, gauze bolsters are applied which are removed within a week, usually along with any surgical drains that may have been placed. Patients are encouraged to walk and return to their activities of daily living immediately following surgery, however strenuous exercise should be avoided. Patients typically return to work in about a week. An elastic garment will be placed over the chest to provide snug compression, and it must be worn continuously for approximately one month. It may take many months before all the swelling resolves and the final results are seen.
