The nose is a complex organ composed of a delicate envelope of skin overlying a support structure of cartilage. Surgery to reshape the nose (rhinoplasty) is extremely meticulous, as it is often a difference of millimeters between a mediocre and an outstanding result. Furthermore, the shape of the nose in relation to the rest of the facial features must always be considered such that a natural harmony is preserved. Some patients elect to change the shape of the nose they were born with, others undergo surgery to fix the result of an injury to their nose.

When I first began to explore the idea of getting a rhinoplasty, I only went to Dr. Blechman for preliminary information since I wasn’t ready to take the plunge quite yet and the consultation was free. Rhinoplasty typically takes two to three hours and is performed under general anesthesia. Various surgical techniques can be used to increase or decrease the size or width of the nose, alter the profile and bridge, refine the shape of the tip, narrow the span of the nostrils, or change the angle between the base of the nose and upper lip. In some cases, rhinoplasty can include techniques to correct difficulties with breathing, such as those caused by a deviated septum. Incisions are concealed inside the nostrils, although an incision may extend inconspicuously across the columella (the column of skin separating the nostrils). In some cases, a small graft of cartilage is sculpted and used to achieve a desired shape to the nose.

Patients go home the day of surgery and are permitted to walk around and commence with light activities. Special tape, as well as a plastic splint, is placed on the nose to protect it and help maintain its shape. These are removed in less than a week along with any stitches in the skin. Many patients will have swelling and bruising around the nose and eyes which can be minimized with cold compresses. It will take a few weeks for the aesthetic changes from surgery to become apparent, although it may take six months to a year for all the residual swelling to resolve, at which time the final result will be evident.

Rhinoplasty complications are uncommon and are usually minor. As an experienced, board-certified plastic surgeon, Dr. Blechman takes a conservative and educated approach to rhinoplasty and will discuss in detail all the risks, alternatives, and benefits prior to performing surgery.
